The Golden Rule: Empty Stomach, First Thing in the Morning
Levothyroxine must be taken on an empty stomach, first thing in the morning, with a full glass of plain water. This is not optional โ it is the single most important rule for effective absorption. Food, especially dairy and high-fibre foods, significantly reduces how much of the medicine enters your bloodstream.
The recommended protocol: wake up, take your tablet, wait 30โ60 minutes, then have breakfast. Difficult for busy mornings โ but essential.
What Interferes With Thyroxine Absorption?
- Milk and dairy (calcium blocks absorption) โ wait 60 minutes before chai with milk
- Calcium supplements โ take at least 4 hours apart
- Iron supplements or iron-containing multivitamins โ 4 hours apart
- Antacids (ENO, Gelusil, Digene) โ 4 hours apart
- High-fibre foods (oats, bran) immediately after the tablet
- Soy products โ soy milk, tofu
- Coffee โ even black coffee reduces absorption
Dose Adjustments: When and How
Thyroid hormone requirements change. The following situations often require dose adjustment:
- Pregnancy โ requirements increase by 25โ50% in the first trimester. Alert your doctor immediately when you discover you are pregnant.
- Significant weight change โ major weight gain or loss may alter requirements
- Ageing โ requirements often decrease in elderly patients
- Starting or stopping certain medications โ especially oestrogen, some heart medicines
Never adjust the dose yourself. TSH monitoring every 6 months (or 6 weeks after any dose change) is essential.
