COPD โ Chronic Obstructive Pulmonary Disease โ affects millions of Indians, yet it remains one of the most under-diagnosed conditions I see in my Kolkata practice. Many patients attribute their breathlessness to age, weight, or "just a smoker's cough" and delay seeking help for years.
The truth: COPD cannot be cured, but it can absolutely be managed. With the right treatment and lifestyle changes, most patients significantly improve their breathing capacity and quality of life.
What Exactly Is COPD?
COPD is a chronic inflammatory lung disease that causes obstructed airflow from the lungs. It includes two main conditions that often coexist:
- Emphysema โ damage to the air sacs (alveoli), making it harder to exhale
- Chronic bronchitis โ chronic inflammation of the airways, causing persistent cough with mucus
The primary cause in India is smoking, but long-term exposure to indoor cooking smoke (wood fire, biomass), pollution, dust, and chemical fumes also contribute significantly โ particularly in women who have never smoked.
Recognising the Symptoms
- Breathlessness on exertion (climbing stairs, walking fast) that has gotten progressively worse
- Chronic cough โ especially in the morning โ with mucus or phlegm
- Wheezing (a whistling sound when breathing)
- Chest tightness
- Frequent chest infections that take longer to recover from
- Fatigue and reduced exercise tolerance
๐ฉบ The COPD Test: Spirometry
COPD is diagnosed by a lung function test called spirometry โ you breathe into a machine that measures how much air you can exhale and how fast. It's quick, painless, and available at Manipal Hospital Mukundapur. If you've been smoker for more than 10 years and have any breathing symptoms, ask for a spirometry test.
Treatment: What Helps COPD Patients
1. Inhalers โ The Cornerstone of COPD Treatment
The most important treatment for COPD is inhaler therapy. Bronchodilators (LABA, LAMA) open the airways; corticosteroid inhalers reduce inflammation in severe COPD. Using the correct inhaler technique is critical โ a poorly used inhaler delivers almost no medication. I always demonstrate correct technique in the clinic.
2. Smoking Cessation
If you smoke, stopping is the single most powerful intervention that slows COPD progression. Nicotine replacement patches, chewing gum, and medications are available โ and they work. Talk to me about a cessation plan.
3. Pulmonary Rehabilitation
Structured breathing exercises and physical activity training improve breathing efficiency, reduce breathlessness, and help patients do more. I provide basic breathing exercise guidance at every COPD consultation.
4. Vaccination
COPD patients are very vulnerable to chest infections. Annual flu vaccination and pneumococcal vaccination are strongly recommended. I advise all my COPD patients to get these every year.
Managing a COPD Flare-Up
A flare-up (exacerbation) is when symptoms suddenly worsen โ more breathlessness, more sputum, sometimes fever. Flare-ups are serious and need prompt treatment. Seek care the same day if you notice:
- Your usual inhaler isn't providing relief
- Breathlessness is much worse than usual
- Sputum has turned yellow, green, or increased significantly
- You have fever alongside breathing symptoms

Blue or grey lips/fingertips, inability to speak more than a few words without stopping to breathe, very rapid heart rate with severe breathlessness โ these are signs of severe respiratory failure requiring emergency care.
